COVID-19 Announcements

Pfizer COVID-19 vaccine receives FDA approval

The U.S. Food and Drug Administration (FDA) has approved the Pfizer COVID-19 vaccine for individuals 16 years of age and older. The approval replaces the emergency use authorization granted by the agency last December.  Full approval means the Pfizer vaccine now carries the FDA’s strongest endorsement of safety and effectiveness. The Pfizer vaccine also continues to be available under emergency use authorization (EUA) for individuals 12 through 15 years of age and for the administration of a third dose in certain immunocompromised individuals.

COVID-19: $100 Vaccine Reward Program

From August 20 through September 6, the Wisconsin Department of Health Services (DHS) is giving one $100 Visa Gift Card to Wisconsin residents ages 12 and older who receive their first COVID-19 vaccine from a Wisconsin vaccination provider. Everyone who is eligible to receive the free COVID-19 vaccine is eligible to receive this gift card, regardless of insurance, ID, or citizenship.

To receive this $100 Visa Gift Card, you must provide some personal information such as name, address, and the date when you received the first dose of a COVID-19 vaccine. For individuals between the ages of 12 and 17, a parent or guardian must fill out the form.

According to DHS, your personal information is private healthcare information under Wisconsin statute section 146.82 and is only collected to verify your eligibility to receive an incentive through this program and to send you the incentive you requested. For more information on your privacy and to submit your information for this incentive, visit https://100.wisconsin.gov/.

Help! What do I do if I lost my vaccination card?
